Performs general nursing care to residents within an assigned unit under the direction of the Nursing Coordinator; Utilizes the nursing process to perform duties of staff nurse in terms of administration of medication and medical treatment within an assigned unit; provides and supports medical care as directed by medical staff pursuant to objectives and guidelines of the nursing department.

REGISTERED NURSE II

Performs general or specialized nursing care to residents within an assigned unit under the direction of the Nursing Coordinator; Utilizes the nursing process to perform duties of staff nurse in terms of administration of medication and medical treatment within an assigned unit; provides and supports medical care as directed by medical staff pursuant to objectives and guidelines of the nursing department.

REGISTERED NURSE III

To provide care and treatment to a variety of residents within an assigned unit; Responsible for administration of medication and comprehensive medical treatment planning; may assist the Nurse Coordinator with supervision and training of nursing staff; provides and supports medical care as directed by medical st aff pursuant to objectives and guidelines of the nursing department.

Responsibilities:
REGISTERED NURSE I

Utilizes all components of the nursing process including nurses notes, Plan-of-Care, admission assessments, and MAR reviews to provide care for assigned residents with physical and mental condition.

Exercises considerable judgment in monitoring, assessing, and implementing medical and nursing interventions of residents within the assigned unit to ensure quality, nursing service.

Counsels residents to components of their Plan-of-Care and courses of action which might be taken to achieve the goals agreed upon at the Plan-of-Care meeting to ascertain their willingness to comply and assure resident’s to “Right-to-Know”.

Receives pertinent data from subordinates relative to physical/psychological changes in the residents and relays information to Nurse Coordinator to determine course of intervention if required to maintain facility standards.

Reports routine and unusual facts and situations to the Nursing Coordinator and gives report, to on-coming shift, of events/occurrences on their shift to provide continuity of care throughout the department.

Makes contact with physicians, specialists, and allied health professionals using professional assessment/judgment skills in relating facts as presented during the consultation to convey accurate descriptive information lending itself to informed medical decision making.

Administers prescribed medications and makes notations, before and after, of effects of medications given to guard against polypharmacy issues, and mismanaged medication regimes and reports these to Nurse Coordinator.

Admits, transfers, and discharges residents as directed by the attending physician to provide for timely medical care or least restrictive housing options.

Supervises nursing staff assigned to their unit.

REG ISTERED NURSE II

Utilizes all components of the nursing process including nurse’s notes, Plan-of-Care, admission assessments, and MAR reviews to provide care for assigned residents with physical, mental and emotional condition.

Exercises considerable judgment in monitoring, assessing, and implementing medical and nursing interventions of residents within the assigned unit to ensure quality, nursing service.

Counsels residents to components of their Plan-of-Care and courses of action which might be taken to achieve the goals agreed upon at the Plan-of-Care meeting to ascertain their willingness to comply and assure resident’s to “Right-to-Know”.

Receives pertinent data from subordinates relative to physical/psychological changes in the residents and determines course of intervention if required to maintain facility standards. May be assigned to provide in-service education.

Reports routine and unusual facts and situations t o the Nursing Coordinator and gives report, to on-coming shift, of events/occurrences on their shift to provide continuity of care throughout the department.

May participate with physicians in making rounds, evaluating and assessing resident’s conditions, communicating information to interdepartmental team.

Makes contact with physicians, specialists, and allied health professionals using professional assessment/judgment skills in relating facts as presented during the consultation to convey accurate descriptive information lending itself to informed medical decision making.

Administers prescribed medications and makes notations, before and after, of effects of medications given to guard against polypharmacy issues and mismanaged medication regimes. Monitors the effectiveness of medication and reports this to Nurse Coordinator.

Admits, transfers, and discharges residents as directed by the attending physician to provide for timely medical care or least restrictive housing options.

Supervises nursing staff assigned to their unit. May assume responsibility as Charge Nurse, in the absence of Nurse Coordinator.

REGISTERED NURSE III

Utilizes all components of the nursing process including nurse’s notes, Plan-of-Care, admission assessments, and MAR reviews to provide care for assigned residents with psychological diagnosis.

Exercises considerable judgment in monitoring, assessing, and implementing medical and nursing interventions of residents within the assigned unit to ensure quality nursing service.

Counsels residents to components of their Plan-of-Care and courses of action which might be taken to achieve the goals agreed upon at the Plan-of-Care meeting to ascertain their willingness to comply and assure resident’s to “Right-to-Know”.

Reports routine and unusual facts and situations to the Nursing Coordinator, gives report to on-coming shift of eve nts/occurrences on their shift to provide continuity of care throughout the department.

Receives pertinent data from subordinates relative to physical/psychological changes in the residents and determines course of intervention if required to maintain facility standards; will be assigned to provide in-service education.

Participates with physicians in evaluating and assessing residents’ physical and emotional states, coordinates implementation of medical regimen.

Makes contact with physicians, specialists, and allied health professionals using professional assessment/judgment skills in relating facts as presented during the consultation to convey accurate descriptive information lending itself to informed medical decision making.

Administers prescribed medications and makes notations, before and after, of effects of medications given to guard against polypharmacy issues and mismanaged medication regimes and reports these to the Nurse Coo rdinator.

Admits, transfers, and discharges residents as directed by the attending physician to provide for timely medical care or least restrictive housing options.

REGISTERED NURSE I

MINIMUM QUALIFICATIONS:
Education: Graduation from a recognized nursing program with either affiliate or postgraduate courses and clinical experience in medical, surgical, obstetrics, pediatrics and psychiatric nursing.

Experience: Experience is limited to that required for registration.

License/Certification: Current license as a Registered Nurse in New Hampshire.

Education: Bachelor’s degree from a recognized college or university with major study in nursing, human services, psychology, or a related field.

Experience: One year of experience as a registered nurse in a residential, nursing home, long-term care facility, or similar setting.

OR

Education: Graduation from a recognized nursing program and possession of an Associate’s degree in nursing, human services, psychology, or a related field.

Experience: Two years of experience as a registered nurse in a setting similar or equivalent to the position assigned.

AND

License/Certification:
1. Current license as a Registered Nurse in New Hampshire.

2. Current certification in CPR.

3. Must be certified by the American Nurses Association or an other nationally recognized certifying entity in a specialty that is consistent with the job accountabilities and appropriate to the institution or agency in which the vacancy exists; e.g., Psychiatric and Mental Health Nurse, Gerontological Nurse, Maternal and Child Health Nurse, Oncology, Wound Care, etc.
