The Clinical Pharmacist serves as a member of the Pediatric Pharmacy team working with the Clinical Pharmacist Lead to provide integrated pharmacy services to Pediatric,! Neonatal and Obstetric patients.
Responsibilities also include:
Rounding with pediatric care team
Performing therapeutic drug monitoring
Verifying physician orders in electronic health record
Overseeing pharmacy support personnel activities related to the accurate compounding and dispensing of pharmaceuticals
Participating in process improvement projects
Serving on pediatric or pharmacy committees
Ensuring cost effective care and quality patient outcomes
Documenting clinical activities
To qualify for this position you must be the graduate of an ACPE-accredited School of Pharmacy with a Bachelors of Science; PharmD degree and residency preferred. Must be currently licensed to practice in the state of NH. Previous health system experience preferred. Must be able to work independently with minimal direct supervision, to set priorities, make critical decisions, respond quickly to emergency requests, handle frequent interrupti! ons, and adapt to changes in workload. Good written and verbal! communication skills are essential.
